Authorization to Walk Home & use REC CENTRE PROGRAMS unescorted 2023.docxComplete and return this form to us if your child is 10 years of age or older and you authorize them to participating in activities at Trinity Recreation Centre.
After school activities 2023.docxComplete and return this form to us if your child is 10 years of age or older and you authorize them to participating in school extra curricular activities during CARE hours.
Authorization to WALK HOME FROM CARE 2022.docxComplete and return this form to us if your child is 10 years of age or older and you authorize them to sign themselves out and walk home alone from CARE.
Authorized Pick up 2023.docxComplete and return this form for any new authorized persons picking up your child. Verbal or written permission must be received before we release a child to anyone who is not authorized.
